Most Health assessments are performed by Patients Usual GP with assistance of Practice nurse.
This health assessment focuses on the needs of people around 45 to 49 years of age who are at risk of developing a chronic disease. The aim of this health assessment is to assist with the detection and prevention of any chronic diseases you may be at risk of developing, to enable early intervention strategies to be put in place to ensure you maintain the best possible long-term health
At this stage in life, your GP may be able to make recommendations which, if adhered to, can help ensure that you avoid many of the health problems faced by your peers later in life.
This assessment involves a 30 minute consultation with our practice nurse, followed by a 15 minute consultation with your GP, and aims to provide tailored recommendations to safeguard your future health.
Patients are eligible for this assessment once between the ages of 45 and 49 (inclusive). Standard gap fees apply.
The Diabetic Risk Assessment is aimed at people aged between 40 and 49 years of age who are at high risk of developing Type 2 Diabetes and utilises the Australian type 2 diabetes assessment tool (AUSDRISK) to assess your risk of developing type
2 diabetes over the next 5 years.
If you have a high result (>12), you are eligible to attend a diabetes risk evaluation with your doctor for further testing and management and to assess your eligibility for a subsidised Lifestyle Modification Program.
Patients aged 75 years and over are eligible for this assessment once a year, and it is bulk billed for patients with a valid Australian Medicare card.
The assessment is conducted by one of our Practice Nurses on behalf of your doctor and then seen by the doctor. This health assessment explores into your holistic wellbeing – from physical health to psychological and social functions.
This yearly assessment involves a 45-minute consultation with our nurse, followed by a 15-minute consultation with your GP. It encompasses various tests, from hearing and vision checks to blood pressure evaluations, nutritional queries, and discussions regarding psychological and social health.
Part of the assessment also involves discussing your living arrangements to ascertain the services available to you for additional help within the home should you need it. It provides a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions to improve health and/or quality of life.
Having a regular heart health check with your GP will help you better understand your risk of a heart attack or stroke.
A Healthy Heart Check involves a 30-minute consultation with your GP and analyses your risk factors through lifestyle discussions and specific tests.
Eligible patients include non-ATSI patients aged 45 years and over, or Aboriginal and Torres Strait Islander patients aged 30 years and over.
If you identify as an Aboriginal or Torres Strait Islander person and have a Medicare card, you can get a free health check every year.
This health check is important. It can help identify whether you’re at risk of illnesses or chronic conditions. It’s much easier to prevent these than to treat them, so catching them early can make a big difference.
If you have a chronic condition, your health check can help make sure you are getting the medication and care you need. You can also ask your doctor about anything you’re worried about or need help with.
Your doctor can only do this health check if you agree. You can have it every 9 to 12 months, and it’s free at any Aboriginal Medical Service or bulk-billing clinic.
After your health check, your doctor can refer you to up to 10 free follow-up services

Expect a thorough check-up including medical history review, physical examinations, and various screenings depending on your age and health. You will be seen by our clinical nurse first who performs the necessary checks. Once seen by our Clinical Nurse, Your Doctor will go through and complete the assessment to give your tailored advice & frequency of health assessments needed.
